Schema-bloc block-diagram problem, can anyone HELP?

I've nearly managed to create the block diagram I want (the attached figure shown here ) . However, I cannot add the disturbance, d + its line, at the second summing junction.

I cannot translate schema-bloc.pdf, can anyone show me how I create the missing disturbance?cascade_control_disturbance.tex
