iText can do it.
Use SimpleBookmark to get the bookmarks from the existing document.
There are different ways to 'extract' the pages. These are two examples:

but there are more possibilities.
You can also manipulate the bookmarks and add the altered bookmarks
to the individual subsections programmatically.
