Newbie to Gramps here.
 
1. My grandmother had a child out of wedlock. This child was brought-up by my grandmother's parents as one of their own (they already had 15 children spanning 21 years so one more made no difference!). She used my grandmothers maiden name.

Add a family to your grandmother with she as mother, and no father, add the child to the family with relation 'birth' . If you know the father, add the father to the family, but make sure the relations between the parents is _NOT_ set to married, but to eg relation type 'wedlock' or however it is called in English.
Next, to the family of your grandmothers parents, add the child also, but now not with relation birth to the parents , but adopted, or some other indication.
In the person editor of this child, add the birth name as name, but in the names tab, include also the other names used (eg maiden name of your grandmother)
 
Make sure to add a source to all the data you have. If you only know this by talking to people, add eg a source 'Interview with my grandmother, 1-1-2008', and link that to this information.
 
2. My great grandparents also had a child out of wedlock together. This child was brought-up by my grandparents after they married but was never 'adopted' by them - he was given/used his mother's family name.

You can do this in two ways.
Make a family with relation unmarried between the two people, and add the child, with relation birth. Then, make a second family between the same two families, now with relation married, and add the child again.
I would however choose to make only one family, and add some events to the family. One event would be the birth of the first child, and another event would be the marriage event. The birth event would show before the marriage event, making clear the birth was before the marriage. One could go further, and add an event indicating when the couple started to live together.
