Hint Refactoring

Folks:

I've updated all of the bitmap sources and modifiers to use the new
hint-mapping mechanism, which seems to be working well - this cleans-up
loose-ends in the pipeline that have been a source of inefficiency /
ambiguity / confusion for a long time.  The plan is to wait awhile and
see that nothing goes wrong, then start working on (the much larger task
of) converting mesh-related plugins.  Evan: this touched all of the CUDA
bitmap plugins too, you'll want to familiarize yourself with these
changes and keep an eye out for regressions.
