Goes BANG when recording - quite regular problem here!

Some javascript/style in this post has been disabled (why?)
I've been experiencing completel failure when recording - only in stereo, no other tracks on playback or record. This seems to happen at least three or four times in a days work. I'm so paranoid now I restart audacity every time I record a new track. This is the crash dump - any advice greatfully received!
